我清楚的知道我应该为我在考试上糟糕的表现负上全部的责任，但是鉴于我的situation太过极端以及太过糟糕，同时delay某某课程对我的影响有非常的大，我不得不尝试一切可能去弥补。我非常抱歉如果我打扰到了你，如果你无法提供任何帮我，我也可以理解。但是我将非常感激如果我能从你这得到一些帮助。

I know exactly I should take the full responsibility to undertake this horrible job I made myself in my exam, since my situation is too extreme and too bad, and delay subject 'XXX' will impact me seriously, so I have to try anything I could try to fix it. I am very sorry if I bother you, but I will be very appreciated if I could get some help from you.

I know exactly I need to take full responsibility for the awful exam result. Despite of that, I am apperling for your help. My situation was too bad as （此处要给出具体的事情，需要事实的，比如医生给的证明） my doctor gave me a medical advice to take a semester break, while I did not to waste any time. Any delay for the time of graduation would cause a serious impact on me and I would like to try to get a second chance from you.
I would be much appreciated if you could offer some suggestions.
