Just wrote the following, I tried to keep it simple, because I was assuming you are not at a very advanced level:

Dear Mr./Ms.

I am writing to you to explain why I did not show up for the English test on this Monday.

In the morning that day, I was riding a bicycle on my way to school, as I usually did. Suddenly I got hit by a car, and my left leg was injured from the accident. I had to go to the hospital, and therefore I missed the exam.

This is basically what happened, and why I didn't attend the test. Sorry about any trouble I might have caused. I would be very grateful if I can have the opportunity to take a make-up exam someday in the future, so that my academic performance would not be affected. Thanks.
