Itisnowwellacceptedthatthepainsuppressiveeffectofmorphineandsurrogatesisrelatedtothei... It is now well accepted that the pain suppressive effect of morphine and surrogates
is related to the interaction of this alkaloid with μ-, δ- and κ-opioid receptors,
located in the CNS and, more precisely, within structures (e.g., spinal cord,
periaqueductal grey matter and thalamus) known for their involvement in the
regulation of nociceptive stimuli. The wide distribution of opioid receptors in
the brain and peripheral organs accounts for the multiplicity of pharmacological
responses elicited by morphine administration. Even if the most efficient drugs
to alleviate severe pain are opioid compounds, their chronic use could be associated
with serious drawbacks, such as tolerance, respiratory depression, constipation,
dependence and sedation. This is very likely related to ubiquitous
overstimulation of brain receptors by these exogenous opioids. In the case of less
severe pain, particularly associated with inflammatory processes, NSAIDs are
currently used. However, they are moderately efficient and endowed with serious
problems in particular cytotoxicity for stomach epithelial cells. Therefore, there
is a need for efficient drugs active on moderate pain that can fill the gap between
the narcotic and non-narcotic drugs. Such analgesics are expected to have a great
interest for the treatment of postoperative pain, pain in children and the elderly,
in patients with respiratory defects or suffering from chronic pain and refractory
to the existing NSAIDs, and from neuropathic pain. In spite of recent progress in
the development of new compounds, there is yet no marketed drug fulfilling
these properties.
