不过得纠正一下楼主的问题，因为本题并无定语从句
前面是it is...that....的强调句
后面that引导的是同位语从句，用来解释decision的内容
所以前后都不是定语从句啊！

选B ,因为第一个空是强调句，在固定结构the same...that...; so...that...; such...that...以及it is/was...that...的强调句型中,要用that
第二空是一个句子中如有两个从句,第一个从句用了关系代词that,第二个从句中的关系代词宜用which;但在平行结构中,应重复同一个关系代词

我认为第二句不是同位语从句，因为同位语从匈与定语从句的区别是同位语从句把Mr.Smith made a decision 删去we should send for a doctor仍为一个完整的句子，但send缺少了宾语
引导同位语从句的that是连词，在从句中不充当任何成份，而引导定语从句的that是关系代词，除起连接作用外，还在从句中充当主语、宾语或表语等。所以我认为第二个空是定语从句
