I am suffering from a health condition that requires an operation. and in need for 2-3 days leave time (from 5th May till 7th May). Attached is the doctor's certificate.

** Depending on my recovery, if it is possible, I may cut short my period of leave and return to work earlier. (i.e. 2 days of leave in stead of 3)
