Each teacher has it's own style, and as that, only some of the things mentioned above can be done.
Hollywood movies are good, but most lessons are shorter than any movie.. The movie would have to be carefully chosen to avoid boredom or funny distractions.. There are all sorts of games a teacher can play with the students, so they can have fun while learning and of course, keep always motivated in class. The raising hand rule, it only applies for larger classes really, I've had a class where I tried that but didn't work, as the students were very shy to give me an answer, so I adopted using a ball and throwing it to random students to get them to answer questions or simply to get them to talk..
Being patient is THE most important key a teacher needs, now, when it comes to lesson planning I think it's really up to the teacher to decide. Different classes have different types of students (visual, kinaesthetic,..) only that class's teacher can really know what to do.. If I were an English teacher, I would try to make the class as interesting as possible. I would read English novels, stories, or tales to my students. I would lead them to play all kinds of games. I would ask them to tell stories about themselves. I would play DVDs of Hollywood movies for them. I would give compliments to those who did well in the class. I would encourage those who kept silent during class to stand up and speak out . I would love and care for each of the students. I would make them all love English. I am sure all my students would like me and love the English class because they found this was the most interesting class they ever had.
