1主句是过去事态，A是表示对现在情况的肯定D是表示对过去事情的猜测
把could only换成 must也可建议你看一看对过去事情猜测的表达方法。
2The woman saw"a large cat "five yards away from her.这句话本身就完整five yards away from her做"a large cat "的定语within是介词如果用它within five yards away from her就是一个介词短语而不是名词短语了，no more than 是不超过的意思相当于形容词，four or five 是四或五的意思，因此C更好
3句子不完整，语境不清楚没法做
4get/be used to 习惯于……to 在这里是介词不是不定式
把could only换成 must也可建议你看一看对过去事情猜测的表达方法。
