摘要：规范的用药交代与指导是开展优质药学服务的前提。在日常的工作中，一些医师由于某些原因使得所开具的处方超过药品说明书规定用量或超出规定的相关适应症，而为数众多的患者对所患的疾病及所需使用的药物知之甚少，这就难免出现因为药品超说明书使用而导致患者出现各种过敏反应乃至药物中毒发生生命危险。为了了解笔者所在医院住院部处方药品超说明书使用的情况以规范医患合理用药、保证用药安全，笔者就所在医院住院药房从2010年3月1号开始至2010年11月31号止收集到的103份药品超说明书使用的处方进行了调查分析。结果发现药品超剂量使用占所收集到的处方的92%，错误用药占7%，护士录入错误占4%。因而医院应着重加强对药品超剂量使用的监管，谨慎对待药品超说明书使用，指导合理用药。相关部门也应立法规范超药品说明书用药，强调药品管理制度和医疗行为规范。

In order to understand how the author hospital where the inpatient super manual prescription drugs used to standardize doctor-patient rational drug use, ensure safe drug use, author will place hospital pharmacy from 2010 March 1, 2010 November 31 check collected 103 copies manual use prescription drug super was investigated.
